Primary Failure of Eruption: From Diagnosis to Multidisciplinary Success

Abstract
Primary failure of tooth eruption (PFE) is a non-syndromic eruption disorder characterized by partial or complete failure of tooth eruption despite a clear eruption pathway and the absence of mechanical obstruction. The condition commonly presents with posterior open bite, reduced vertical alveolar development, and altered occlusal and skeletal relationships. Heterozygous variants in the parathyroid hormone 1 receptor (PTH1R) gene have been identified as the principal genetic cause of PFE. Importantly, misdiagnosis may lead to inappropriate conventional orthodontic treatment, which is typically ineffective in these patients. Application of orthodontic forces to affected teeth frequently induces ankylosis, resulting in minimal tooth movement before ankylosis to the alveolar bone, progressive occlusal deterioration, and compromised future restorative or surgical options.
Successful management of PFE therefore requires early and accurate diagnosis, meticulous treatment planning, and close collaboration among specialists. Genetic confirmation, avoidance of inappropriate orthodontic force application, and careful selection of age-appropriate restorative or surgical interventions are critical to achieving stable functional and esthetic outcomes.
This presentation reviews the diagnostic criteria of PFE, encompassing clinical, radiographic, and genetic considerations, and outlines contemporary management strategies. Through clinical cases of varying severity, it demonstrates individualized, multidisciplinary treatment approaches integrating orthodontics, prosthodontics, and surgical procedures.
